Advancements in Neuropsychiatric Services in Malaysia

Neuropsychiatric services in Malaysia have experienced significant growth and development in recent years, contributing to the overall improvement in the healthcare landscape. Within the public sector, there are currently seven dedicated neuropsychiatrists who play a crucial role in addressing the complex needs of patients with neuropsychiatric conditions. Additionally, several neuropsychiatrists are providing their expertise in the private sector, ensuring accessibility to a broader spectrum of individuals seeking specialised care.

To further enhance the capabilities in this field, there are presently six clinical fellows specialising in neuropsychiatry. These clinical fellows are a promising addition to the field and are set to be posted in hospitals across key locations, including Ipoh, Sungai Buloh, and Penang, in a well-structured time sequence. This expansion of the workforce, combined with their training, holds great promise for ensuring that the population has access to the expertise needed for managing complex neuropsychiatric conditions effectively.

Commitment in Providing Comprehensive Neuropsychiatric Care

Neuropsychiatrists work closely with a multidisciplinary team of healthcare professionals, including neurologists, neuroradiologists, neurorehabilitation physicians, nuclear medicine physician, neuropsychologists, speech therapists, occupational therapists, and other allied healthcare providers.

This collaboration results in a comprehensive diagnostic service that leverages the latest advancements in medical knowledge and technology. Together, they provide evidence-based interventions and therapies, ensuring that patients receive the highest quality care in the field of neuropsychiatry. This comprehensive and collaborative approach reflects the commitment of Malaysia's healthcare system to addressing neuropsychiatric conditions with the utmost care, expertise, and compassion.

The Gaps between Neurology and Psychiatry

Neurocognitive disorders

Neurobehavioural issues due to neurological conditions

Neurobehavioural issues due to neurological conditions

  1. Yound onset dementia (YOD): Mild  (i.e., mild cognitive impairment) and major neurocognitive disorders (i.e., dementia) in young people including rare and atypical dementias e.g., atypical Alzheimer's disease, language variant of FTD etc.
  2. Cognitive impairment associated with psychiatric illness

Psychological and behavioural issues due to neurological illness and acquired brain injury, e.g.:

  • Traumatic brain injury (TBI)
  • Movement disorders e.g., Parkinson's disease, Huntington's disease
  • Autoimmune and demyelinating diseases e.g., anti-NMDA receptor encephalitis, multiple sclerosis
  • Metabolic diseases e.g., Niemann Pick disease, phenylketonuria, leukodystrophy

Functional neurological disorders

Neurobehavioural issues due to neurological conditions

Neuropsychiatry of Movement disorders

  1. Functional cognitive disorder (not in DSM5 but under extensive research)
  2. Functional movement disorder such as dystonia, tremor, gait and so on
  3. Functional seizure disorder a.k.a. dissociative seizure or non epileptic psychogenic seizure
  4. Other neurological conditions involving anaesthesia/sensory loss, speech symptoms, swallowing symptoms, paralysis etc.

  • Parkinson's disease (non-motor and medication related manifestations)
  • Medication/drug-induced movement disorders i.e., tardive syndrome
  • Tic disorder and Tourette syndrome
