Title: "Caught Betwen Response and Cognition: Two Cases of TRD Managed with ECT and tDCS Cross-Tappering"

Synopsis:
We present two cases of super treatment-resistant depression (TRD): one predominantly with severe somatic symptoms and another with profound melancholic features. Both patients developed significant cognitive impairment and faced major logistical barriers in accessing ECT, the most effective treatment for their condition. Their clinical course was further complicated by repeated breakdowns of an aging ECT machine, leading to interruption of maintenance treatment and subsequent relapse. Following repair of the machine, both patients underwent a second course of ECT; however, attempts to further space maintenance sessions were unsuccessful. Long-term stabilisation was eventually achieved with the introduction of transcranial direct current stimulation (tDCS). These cases highlight the challenges of sustaining ECT services in resource-limited settings and suggest that home-based tDCS may serve as a valuable adjunctive strategy, particularly for remote patients with poor accessibility to ECT services.

Synopsis:
A 42-year-old Malay man with no prior psychiatric history was diagnosed with young-onset Parkinson’s disease (YOPD) in 2022, following a one-year history of progressive right-sided rigidity, bradykinesia, and painful muscle spasms. He initially demonstrated a good response to levodopa therapy; however, his disease course was complicated by early motor fluctuations, including wearing-off phenomena and levodopa-induced dyskinesia. Over time, he developed features of dopamine dysregulation syndrome, characterized by compulsive self-escalation of dopaminergic medications to supratherapeutic doses. Subsequently, he presented with recurrent neuropsychiatric symptoms, including visual and auditory hallucinations, paranoid delusions, and somatic passivity phenomena, temporally associated with dopaminergic overuse and amantadine initiation. These episodes were accompanied by behavioral disturbances, including aggression and risk-related behaviors. He also reported symptoms of depression and anxiety, including low mood, guilt, hopelessness, and passive death wishes, in the context of significant psychosocial stressors such as divorce, bereavement, unemployment, and financial instability. These symptoms led to psychiatric admission, during which his initial presentation of reduced engagement raised the possibility of catatonia. Antipsychotic treatment was initiated, with subsequent transition to clozapine, resulting in significant improvement in both psychotic symptoms and dyskinesia. Diagnostic complexity arose due to caudate nucleus atrophy observed on neuroimaging, raising suspicion for Huntington’s disease; however, this was excluded through genetic testing. Notably, genetic analysis identified a glucocerebrosidase (GBA) gene mutation, supporting a diagnosis of GBA-associated YOPD, and cognitive assessment revealed mild impairment. This case illustrates the complex interplay between neurodegenerative disease progression, treatment-related complications, and psychosocial stressors in YOPD, particularly in the presence of a GBA mutation. It highlights the challenges in managing dopamine dysregulation syndrome and medication-induced psychosis, as well as the potential role of clozapine in addressing both psychosis and dyskinesia in such patients.

This is a 20yo young gentleman with underlying periventricular leucoencephalomalacia, intellectual disability, epilepsy.
He was referred to us to manage his difficult behavior . This case highlights the indication of pharmacogenomics to help determine efficacy of treatment as a type of precision medicine. This is in line with the fact that this gentleman shown poor response to multiple types of treatment.

Synopsis:
At the Crossroads of Medication, Mind, or Movement Disorder? presents a diagnostically complex case of a 57-year-old man with long-standing schizophrenia who developed progressive parkinsonism and global cognitive decline in the context of chronic exposure to oral and long-acting injectable antipsychotics. Following treatment interruption, he manifested a striking combination of asymmetric parkinsonism, dystonia, dyskinesia, cerebellar signs, abnormal ocular movements (round-the-house eye signs), gait impairment, and rapid loss of functional independence. Cognitive impairment was profound, precluding formal objective testing (NUCOG), with behavioural assessment (BATCH) demonstrating severe impairment across all domains. Neuroimaging revealed small-vessel disease with multiple chronic lacunar infarcts, microhaemorrhages, and cerebral atrophy, further blurring the distinction between drug-induced parkinsonism, tardive syndromes, Parkinson-plus disorders (PSP/MSA), and vascular parkinsonism or dementia. In this context, functional imaging—particularly FDG-PET and TRODAT (DAT) scans—is crucial in differentiating neurodegenerative from medication-related pathology and guiding management decisions, underscoring the need for a pragmatic, multidisciplinary neuropsychiatric approach that prioritises potentially reversible contributors while closely monitoring disease progression.

A case study of a patient presented with clinical symptoms resembling frontal lobe dysfunction in the absence of obvious neurological deficits had a relatively normal structural MRI brain in which later diagnosis was anchored for early onset bvFTD as supportive features shown in FDG-PET imaging. A clinical scenario showcasing the importance of understanding clinical symptoms presentation, neurocognitive domains representation and structural and functional neuroimaging profiling for consideration of concordance upon diagnostic conundrum.

Synopsis:
This CME presents two case studies of patients with schizophrenia on multiple antipsychotics. The parkinsonism observed was initially presumed to be iatrogenic; however, the Tc-99m TRODAT-1 SPECT scan scan proved useful in distinguishing drug-induced parkinsonism (DIP) from idiopathic Parkinson’s disease (iPD). Clinically, differentiating between the two conditions can be challenging.

Synopsis:
A 33-year-old man with long-standing untreated post-traumatic epilepsy presented with a week of severe behavioural disturbance, including agitation, destructive behaviour, hallucination-like symptoms, and marked disorganization. This occurred on top of years of progressive cognitive decline following untreated Epilepsy post-TBIs. During admission, he remained confused and difficult to manage despite medications, which made the possibility of non-convulsive seizure activity of the differential. Nevertheless, with optimization of ASM, condition remain the same. Thus, decision for ECT made and led to significant behavioural improvement, though his cognitive deficits persisted. The case underscores the complex overlap between chronic untreated epilepsy following TBI and neurocognitive decline, and severe neuropsychiatric symptoms as well as the role of ECT as an adjunct when behavioural disturbances become unmanageable.

Synopsis: This is a case narrative of a very interesting lady who initially presented with depression but the inconsistency and incongruency of her symptoms raised the suspiciousness for element of functional neurological disorder. A truly multidisciplinary case discussion with the concept of "drain the symptoms dry" in teasing out the gist and essence of a complex multisystem involvement presentation of a "software" problem. This case shone the lights onto the significance of clinical hypervigilance as well as the judicious use of clinical questionnaires during the clinical assessment.
